Personal Information and Documentation

Applicants must provide the following information or documentation, as applicable: Full Name, Maiden Name, Current Address and Phone Number, Social Security Number (SSN), Original Birth Certificate or Proof of Birth, Adoption Decree

Family Tribal Lineage and
Certificate of Degree of Indian Blood

Applicants must provide a Family Tree (including Parentage, Tribal Affiliation, and Degree of Blood) and a Certificate of Degree of Indian Blood (CIB)

No Affiliation or Enrollment
with Another Tribe

Applicants must provide documentation showing they have relinquished membership in another tribe if previously enrolled or a statement confirming they are not currently enrolled in any other tribe.

 Steps to Enroll

    • Full Name (including AKA or Maiden Name)

    • Current Address (Physical and Mailing)

    • Current Phone Number

    • Social Security Number (SSN)

    • Original Birth Certificate (or Baptismal Record, Adoption Birth Certificate, or Adoption Decree)

    • Family Tree (indicating Parentage, Tribal Affiliation, and Degree of Blood)

    • Certificate of Degree of Indian Blood (CIB)

    • Certificate / Letter of Relinquishment (if previously a member of any other Alaska Native or Native American Tribe)

  • Download and fill out the Sun’aq Enrollment Application

  • Return or completed application and documentation to the Sun’aq Enrollment Manager, Traci Marsh (in-person or by mail).

FAQs

When will I know if I will be officially enrolled in the Sun’aq Tribe?

The Sun’aq Tribe of Kodiak enrollment committee meets quarterly. When submitting your application, please inquire when the next enrollment committee meeting will take place. Once enrollment is confirmed, you will receive instructions for obtaining your Sun’aq Tribe of Kodiak ID and enrollment number.

How do I know if I am already enrolled or if my family is enrolled in the Sun’aq Tribe?

Who is eligible for enrollment?

The following are eligible for enrollment: any person lsited on the Base Roll (anyone who was listed as an original shareholder of the Sun’aq Tribe of Kodiak on the 1971 ANCSA rolls); any blood descendant or adoptee of a Base Roll member and their descendants. Enrollment of other persons of Native descent with biologic lineal historic or ancestral ties to the Sun’aq Tribe of Kodiak is at the sole discretion of the Tribal Council and Enrollment Committee. All such persons shall be listed on the Supplemental Roll.
